This form will be used to measure the personal progress that was made over the entirety of the 3 week Reboot21 program. You will need a measuring tape, a scale, and a camera to complete it.

Upload a picture of your feet standing on top of the scale. Don't worry you're information is confidential between you and the coaches.
Waist Measurement (in inches)
Measure at the smallest point of your waist, usually just above the naval.
Chest Measurement (in inches)
Measure all the way around your chest and back on the line of your nipples.
Hips Measurement (in inches)
Measure around the widest part of the hipbones.
Rear End Measurement (in inches)
Measure around the body at the widest part of your rear end.
Thigh Measurement (in inches)- Right
Measure around the fullest part of your upper right leg while standing.
Thigh Measurement (in inches)- Left
Measure around the fullest part of your upper left leg while standing.
Calf Measurement (in inches)- Right
Measure around the fullest part of your right calf.
Calf Measurement (in inches)- Left
Measure around the fullest part of your left calf.
Upper Arm Measurement (in inches)- Right
Measure above your elbows around the fullest part of your right arm.
Upper Arm Measurement (in inches)- Left
Measure above your elbows around the fullest part of your left arm.
